It's a faulty part to start with, based on the fact that this seems to be a pretty common occurrence.

The only thing I can say is yours seemed to fail at a lower mileage than the majority of the cases that I've read about, but I have seen a few that failed at lower mileage as well, so you're not alone.

Thanks for all the replies.

Follow-up on the repair: The tech did not look at the vehicle after returning it on the day of 1st repair completion (took 3 weeks to complete). After 2 weeks I questioned why there was still a rattling in the gearbox after the initial repair and why it wasn’t completed. Apparently the tech forgot the bolt on the drive shaft or did not torque it down. On the same day that I brought up this issue my vehicle repair was completed (5 weeks total).

Tech notes (verbatim): Replaced throw out bearing, release fork, pivot fork rides on and pressure plate
Parts used:
· FP number 44022AA123 GASKET(CR)
· 44011AG000 GASKET EXH PIP
· 30210AA760 COVER COMPLE
· 30502AA051 BEARING CLUT
· 30531AA220 LEVER CLUTCH
· 30537AA061 PB000955 PIV
· 35025CA000 COLLAR

Things I noticed after repair:
· Clutch pedal is now smooth and there’s no longer a crunch at the end when pressing down on clutch.
· The RPM does not drop as fast when shifting gears
· At the 750 to 1300 RPM, the engine no longer wants to die or sputter (I don’t need to rev higher)
· No more clutch squeak

I so excited to have my car back! I hope this will be helpful information for those who are experiencing the same clutch fork/throwout bearing problem.
