Posted this picture on another forum last night:



Immediate response was "Your going to have much bigger problems if you drop those bolts down intake holes, I would advise some paper towels in intake."

So you shove paper towels (or rags) down all the holes, forget to pull them out and boom, something not fun happens.

The same way a surgeon leaves something behind in a patient. Things like that happen on occasion due to a lot of factors.

If it can happen in an OR with multiple sets of eyes on the patient, it can definitely happen in a garage with one set of eyes on the project.

According to the Center for Auto Safety, a recall issued last fall by Toyota Motor Corp. and Subaru Motor Corp. to address a valve spring issue that caused the engine to stall may not have cleared the cars back to 100-per-cent health.
Tales of stalling and engine knocking are still allegedly being reported.
The automakers recalled 411,000 vehicles globally, including 140,249 from Subaru and 80,000 from Toyota in the U.S.
Canadian numbers haven’t been made available for Toyota, but 19,164 Subarus were recalled nationally.
The Washington D.C.-based non-profit consumer advocacy group told Automotive News it believes the repair itself is busted and suggests the NHTSA (National Highway Traffic Safety Administration) might want to take a look-see for themselves.

There numbers don't add up. These are the numbers I've seen for this:

Vehicles recalled: About 165,000 cars and SUVs, including:
2012 to 2014 Subaru Impreza sedan
2012 to 2013 Subaru Impreza hatchback
2013 Subaru XV Crosstrek SUV
2013 Subaru BRZ coupe
2013 Scion FR-S coupe

No way Toyota has 80,000 cars recalled out of that list.

My Buddy had a shop work on his GTR. They removed some parts for powdercoating. They left rags in his intake and installed the intake in. He didn't drive the car other than into his trailer, drove 1500 miles 1 way for a race. Lost the turbos first time he got into boost. 3000 mile round trip race weekend down the trash, plus then had to pull the engine to get new turbos.

Not to mention they are stating that the recall action is at fault for the new failures (as in the springs themselves) not that it is a totally different issue not related to the spring. Yes, the workmanship during the recall task is faulty. No the spring repair itself is not at fault.

Exactly. I can fish a magnetic bolt out of a hole, I can't fish a rag out of a catalyst.

I had to get a non magnetic bolt out of a catalyst once, about 18" down around a bend. That is the only hole I shove a rag down to this day. OM642 combination box with the top elbow removed. That was a fun one

We put painters tape over stuff we don't want something to fall down if we are worried about it. When you cover the bolt holes, the bolts won't go in either ahaha. So thats a double check beyond the bright colors.

It was probably covered up by a Maine Coon cat, and the mechanic didn't notice the shop rag under the cat when he lifted it off the cover. Could happen.
