Mine was total stupidity, too. Was trying to put a rounded edge on a 1x2" piece of oak moulding, and too lazy to setup the router table. Flipped the router upside down, handguided the wood to the exposed blade... and it caught a knot and pulled my hand right into the blade. Happened so fast; the piece of wood was across my shop like a bolt of lightning and my hand felt like something bumped it.. that's all.. then... well.... I'll stop there. Folks might be eating.
