From everything I have read/heard, he had a few defective parts on his Flex-Fuel Set-up he was selling (not his fault). Caused Fuel to leak and a car to catch fire. He fixed the issue as soon as it was discovered, but having your part play a roll in a car catching fire can be hard to come back from.
