did 02 sensors screw in by hand ? then torque to correct level. if you forced them then it can strip threads or damage sensor
Tips for changing header
1. Always use new gaskets, if you re-use the old ones it will leak causing 02 sensors to read incorrectly
2. Run a thread tap or chaser (M18 x 1.5) through you new header 02 bungs to clean them up , 02 sensors should screw in by hand. If you force them they are easily damaged and the threads on 02 sensor easily stripped.
3. Put high temp anti-seize on header bolts and 02 sensor bung threads, DO NOT get any grease or anti-seize on sensor tips, once contaminated they are stuffed.
4. Leave the 02 sensors in the stock header until you remove it from car then remove sensors. Just disconnect the plugs an tethers on wires as per picture below.
5. Black plug to black socket, grey plug to grey socket. and remember to plug both back in.
6. Use correct torque for bolts and o2 sensors, re-torque after a heat cycle.

Hey thanks very much for your help but you already helped me a few months ago when I posted about this issue on the forums!
I have no leaks, because I had it smoke tested multiple times. Both O2 sensors already replaced, brand new gaskets replaced twice already (second time with thick Grimmspeed gaskets). I ran thread tap through O2 sensor holes, used anti-seize on O2 sensors, everything is perfectly installed, torqued perfectly, yet I still get the code (it goes on and off totally randomly but it's on more than off).
And yes, this happened after the install of the OFH. I never got it on stock tune or stage 1, everything was good. I already emailed the whole story to Shiv, he just hasn't replied back yet after I sent him the data log.
There was an issue in the beginning with the OFH's flange not being flat at the overpipe connection and that showed an obvious leak when smoke tested, but that's been fixed and I still have the code.
